I hope this letter finds you in good health and enjoying good vision. This letter is to inform you that I have retired from active practice. It has been a great pleasure providing for your vision care needs over the years.

In accordance to Ontario Regulations and the Optometry Act, I have transferred all my patient records to another Optometry Clinic. Eyecare Kingston has kindly accepted responsibility for my clinical records. In addition, I greatly appreciate that Dr. Michael Lewis, Dr. Andrew Lewis and associates have agreed to continue to provide eye care to any of my patients who wish to see them.

If you plan to continue your care at Eyecare Kingston, I suggest you use their online forms to register as a patient and/or book an appointment. Although I personally recommend Eyecare Kingston, if you prefer to see another optometrist your records will be available for pickup at Eyecare Kingston.

I have greatly valued our relationship, and thank you for your loyalty and friendship over the years. Best wishes for your future health.
