What to Bring

Please be sure to bring the following to your appointment:

A method of payment - fees apply for all exams. Please refer to our Pricing Policy if you have any questions about our fees.
A face mask - All patients must wear a mask as per provincial mandate for Health Care Providers.
Your Ontario Health Card and other vision insurance information, if applicable.
Your current pair of prescription glasses.
Your contact lenses and the box with the prescription and brand on it.
A list of current medications.
A pair of sunglasses, your eyes may be sensitive to light after dilation.

Things to Remember:

We make every effort to stay on schedule and hope you will too, please arrive 10 minutes before your appointment.
A twenty four hour notice is required for cancellations.
Parking is available at the front of the building and around the side.
